OUR STORY
Zynith Health Care & Associates is a family-owned medical care center dedicated to providing you with the highest quality, cost-effective medical care. We accept some medical health insurance plans and also cater to those without. For patients with no medical insurance or who have a higher medical insurance deductible, we offer a monthly subscription. Our monthly membership plans are age-based but affordable.
They include:
no copay, no deductible, unlimited office sick visits, same or next day appointments, annual physical exam, telemedicine and discounts on labs.

STEP 1
REQUEST AN APPOINTMENT

You will receive an email and/or text message to complete patient registration forms prior to your scheduled visit.
HOME VISIT
You will receive an email and/or text message to complete patient registration forms prior to your scheduled visit. You will also receive a link to access your visit minutes prior to your scheduled telehealth appointment. You may access your appointment from any convenient location of your choosing.
TELEHEALTH
STEP 3
AFTER YOUR VISIT
After your visit, any prescribed medication except narcotics will be sent to your local pharmacy electronically. Visit information may be sent to your primary care provider (PCP) upon request. You can access your visit information through our patient portal after accepting the email invite sent to you for access.
